Health Home is a network of community-based Medicaid care management programs that serve people with serious and complex health needs. At Housing Works, our Health Home program has served over 3,000 clients and employs over 80 staff. Our Health Home clients are more likely to be enrolled in primary care; be virally suppressed; and have higher rates of screenings for mental health, diabetes, and blood pressure than the general Medicaid population. We are fighting to restore state funding for this program and will oppose any further cuts.
